Coverage briefing

Ask what your plan covers for this use.

Suppose a service says it accepts insurance. You want to know what your own plan would pay for the proposed medicine.

Take a specific question to the plan

Ask the person treating you to write down the medicine name and why it is proposed. Give both to your insurer when you ask about payment.

For ordinary Medicare drug coverage in September 2026, weight-loss treatment remains an excluded use. For diabetes or a different accepted medical purpose, prescribing that same medicine may open a route to coverage.

Under September 2026 rules, possible Medicare coverage still depends on your own plan. Ask whether it includes this medicine and must approve a request before paying.

CMS · possible coverage is not a payment promise

Checked September 26, 2026 for this claim.

The plan may call its medicine list a formulary. It may also ask to review a request in advance, called prior authorization. Ask how those checks apply.

Separate the decisions

Treatment discussion

Would this treatment help with my health problem? Ask the person treating you.

Payment discussion

What would this plan pay for this product and use? Put that question to the insurer.

  • Is this exact product on my plan’s medicine list?
  • What information does the plan need from the person treating me?
  • What is my cost if the plan agrees?
  • What happens if the request is declined?

Choose the right coverage briefing

The Medicare and Medicaid pages take different paths. For workplace or private cover, take the same product-specific questions to the plan.

Keep the written reply and the name of the plan you asked. Bring any unclear terms back to the conversation before committing to a care fee.
