# A state plan needs a state answer.

Suppose a national headline sounds promising. Your next question is much smaller: what does the plan on your own card cover?

## Take the treatment name to the plan

GLP-1 medicines copy one of the body’s chemical messages that helps control hunger and blood sugar. A chemical message of this kind is called a hormone.

Under September 2026 rules, Medicaid’s drug-payment program requires coverage of these medicines for diabetes. States can choose whether to cover them for obesity, a disease involving excess body fat.

Ask the plan about the proposed medicine for the diagnosed condition. Use the name of your own plan rather than a general question about peptide programs.

## A new national program does not confirm your coverage

BALANCE is a voluntary program in which Centers for Medicare & Medicaid Services (CMS) negotiates weight-loss medicine prices and coverage terms for Medicaid. In September 2026, states could join, but the official program page named none as participants.

The September 2026 BALANCE rules did not promise that any particular patient’s medicine would be covered.

Keep a news headline and a plan’s written answer in different places. The answer you need names the product and the use.

## Ask for the next practical step

- Is this medicine covered for the condition on the request?

- Does this answer apply to the plan I am enrolled in?

- Is prior approval needed before the plan pays?

- What must the person treating me send to the plan?

- If the request is declined, what review process can I ask about?

Prior approval puts a review ahead of payment. Ask the plan what it needs to decide on the request.

## Keep this thought

National announcements are background. The product-specific reply from your own plan is the useful document.

This is a guide for thinking through care and its costs, with no appointments or medicine to offer.
